To join our preventative maintenance program, please complete, sign & detach the form below. Return the detached portion with a check made out to Ray Gidich Heating & Air Conditioning, Inc. and your agreement will start the day that we receive your payment and signed contract. If you prefer to pay via credit card, mail or email your signed enrollment form to billing@raygidich.com and an electronic invoice will be emailed to you for payment.
Residential Maintenance Program Enrollment Form
Residential Maintenance Agreement Terms & Conditions:
- This maintenance agreement is for single residential units up to and including 5 tons. (Excluding Oil Furnaces)
- This maintenance agreement includes one (1) heating Clean & Check and one (1) cooling Clean & Check per system, per year.
- Our Clean & Check includes a full inspection of the heating or air conditioning equipment for that season, any parts that need to be replaced will cost extra and the homeowner will be informed before replacing. The 20% discount applies for such parts.
- Any media type filter replacements will be an extra cost. (20% discount applies)
- Any water panels in humidifiers are an extra cost. (20% discount applies)
- Homeowner receives 20% off repair parts; excluding refrigerants and equipment installations.
- No additional discounts apply.
- Regular Equipment Maintenance is required to validate manufacturer equipment warranties.
- Homeowner will be contacted by Ray Gidich Heating & Air Conditioning, Inc. to schedule. We will work with your date preferences when scheduling your Clean & Check during our normal working hours.
- Any service work needed will receive preference in our work schedule and performed during our normal work hours.
- Ray Gidich Heating & Air Conditioning, Inc. will attempt to render prompt & efficient service as provided for under the terms of this document; however it is expressly agreed that Ray Gidich Heating & Air Conditioning, Inc. shall in no event be liable for damage or loss caused by delay or any loss arising out of the performance of this agreement.
- This agreement may be terminated by either party upon 30 days written notice. A refund for the remaining contract period will be made on a pro-rated basis with deduction for work already completed/discounted.
- Ray Gidich Heating & Air Conditioning, Inc. reserves the right to reject any agreement if, on inspection by a service technician, equipment is found in such condition that service will be unsatisfactory to both parties.

- Check Thermostat Operation
- Check & Clean Pilot/Ignition Sequence
- Check & Clean Main Burner Operation
- Check Fan Switch
- Check Primary Limit Operation
- Visually Inspect Heat Exchanger
- Visually Inspect Blower Wheel
- Check & Oil Motor (if applicable)
- Replace standard Filter (Media Type Filters at Add’l Cost)
- Check Furnace Temperature Rise & Gas Pressure
- Check Limit Switch Operations
- Check Humidifier Operation (if applicable)
- Operate the Furnace Through a Complete Cycle
Cooling Clean & Check:
- Check Thermostat Operation
- Re-Set Thermostat
- Inspect Condition of Fan Motor
- Inspect & Lubricate Blower Motor (if applicable)
- Inspect Condensate Drain & Flush
- Inspect Condenser Coil & Wash
- Inspect All Electrical Connections in Condensing Unit
- Check Service Valves & Service Port Caps
- Check Compressor Voltage & Amp Draw
- Replace Standard Filter (Media Type Filters at Add’l Cost)
- Check Temperature Drop Across Evaporator
- Check Contactor Contacts for Voltage Drop
- Cycle System Through a Complete Cycle
